Donations

As the largest volunteer hospice in the nation, Fox Valley Volunteer Hospice does not receive any government funding and relies solely on donations from the community to continue to provide all services free of charge.  Your gift will allow us to continue to serve as the recognized voice of hospice, to ensure access to quality hospice care while supporting families before and after death and to educate the community.  

With your donation, you become part of the growing hospice family.  By making a contribution, you will have the satisfaction of knowing you are making each day better for our patients and their families.  All donations are tax-deductible.

Giving options include:

• Single Gift
  A gift of any amount will enable us to continue to provide our services throughout the community.  Making a single donation is one of the easiest ways to show your support for Fox Valley Volunteer Hospice.  You can make a gift using a check, money order or Visa/MasterCard.  To use a check or money order -- or make a memorial gift --  please use this form or you can securely donate online with a credit card.
  


• Monthly Gift
  Join our monthly giving program and have your donation spread out over time.  You can securely set up your pledge online and have your gift of $20, $50 or $100 automatically donated to FVVH each month.

• Memorial Gift
  Memorial gifts are a way to pay tribute to an individual’s life.  Your friend, colleague or loved one’s family will be notified of your thoughtfulness through a personalized acknowledgement card.  You may mail a memorial gift using this form or make your memorial gift online (when donating online, please also send an email to info@fvvh.org, giving us the name of the person that you're giving in memory of, as well as the name and address of the family member to be notified).
  


• Planned Giving-The John Grotberg Legacy Society
  Fox Valley Volunteer Hospice has been blessed by individuals who have remembered us in their estate planning.  There are many ways your family and Fox Valley Volunteer Hospice can receive income from a trust you set up to benefit both.  Your bequest will ensure the needs of persons with life-threatening illnesses, their families and bereaved individuals are met for many years to come. • Workplace Giving
  If your employer engages in a workplace giving campaign, you may be able to designate your payroll-deducted donations to Fox Valley Volunteer Hospice.  We are a qualified 501(c)(3) organization that accepts tax-deductible donations.  If you are donating through a Combined Federal Campaign (CFC), designate your donations to us by using our participant number 8217.
